This position, as part of an interdisciplinary team, performs comprehensive assessment working towards meeting the behavioral needs of each patient and help provide overall quality of care and quality of life.
* Perform social work assessment of the patient at admission and upon change of condition; create, review and update care plan and progress notes.
* Provide direct psychosocial intervention.
* Assist resident's family in coping with physical illness, disabilities of the resident, and potentially the grieving process.
* Complete timely MDS submissions.
Pay is $15+ per hour DOE. Direct placement opportunity.
A Bachelors degree in Social Work or a related field and 2+ years experience in the health care or social work field required. Must be able to effectively communicate with families.
